Who is eligible for EFA?

Client must be certified as Ryan White eligible. Eligibility consists of:

• HIV+ diagnosis of HIV or AIDS
• Income under 300% of federal poverty guidelines, based on gross income
• Residents of Fairfield County

• Medical Fee for Services
• Lab Fees
• Medications and co-pays
• Health Insurance Premiums
• Utilities
• Emergency Rental Assistance
• Medical Transportation

 Applicants must apply for financial assistance through with MFAP's case manager. If you do not have a case manager please call 203-855-9535 to  determine your eligibility. 

Provide the following information:

• Any entitlements (e.g. SAGA, SSI, SSD, TANF, VA, etc.)
• Any income from employment.
• If you are self-employed we need a notarized letter documenting what income they’re receiving and from what source(s).
• Income from all household members (e.g. husband, wife, partner, roommate, children (18 & Up).

• Documentation of HIV status and a CD4/VL done within the last six months
• Identification Card
• Proof of medical coverage (if any)
